The Long Term Care Coordinating Council (LTCCC) is charged with responsibility to advise, implement, and monitor community-based long term care planning in San Francisco and facilitate the improved coordination of home, community-based, and institutional services for older adults and adults with disabilities. Over its ten year history, it has launched a number of policy and planning initiates and carries out its mission with a number of active Workgroups.
The Mayor appoints 32 of the 41 membership slots. The additional 9 slots are represented by various City and County departments. Membership categories were created to ensure representation from a variety of consumers, advocates, and service providers. A list of current members and meeting dates is available on www.ltcccsf.org as well as past plans and publications.
This LTCCC provides stakeholders with a place to share information, discuss issues and formulate solutions. The Council general meets on the 2nd Thursday of the month from 1:00 to 3:00.
